Beginning July 2, Medicare providers who submit paper claims must use Form CMS-1500 (08-05). Claims submitted using Form CMS-1500 (12-90) will be rejected after that date.

In March, CMS had instructed Medicare contractors to continue to accept the Form CMS-1500 (12-90) version due to reports of an incorrectly printed version of Form CMS-1500 (08-05) being sold by some vendors, but the problem is reportedly not as widespread as previously suspected. This requirement has no bearing on the implementation of the NPI.
